Breast augmentation without general anaesthetic: what the Preservé method changes – and what it still has to prove

For some years now, interest in aesthetic surgery has been growing in procedures that are less invasive, mean less downtime and still deliver clinically convincing results. The Preservé method from Motiva is one of the most carefully considered attempts so far to meet this aim in breast augmentation.

The technique

The basic principle differs fundamentally from classic techniques. Instead of cutting through tissue with surgical instruments, an inflatable balloon is introduced through a narrow, blunt channel. It gently pushes the surrounding breast tissue to the side – creating space for the implant without involving the muscle or disturbing the tissue. The implants themselves, the Motiva Ergonomix, are designed to be particularly soft and flexible, which is what makes placement through a small incision possible in the first place.

The procedure takes around one hour, is performed under local anaesthetic and can usually be managed well after surgery with ibuprofen. Many patients are mobile again in the evening, driving is possible after 48 hours and light exercise after one week.

What this means clinically

The reduced tissue disruption has concrete consequences: a shorter healing time, less pain, a faster return to everyday life. For women who are heavily committed through work or family, this fundamentally changes how a procedure is weighed up. At Innerform we perform the method in our own operating theatre in Berlin Mitte. One patient who stayed with us in particular: for years she had wanted breast augmentation – general anaesthetic was a line she would never have wanted to cross. With Preservé, that line was removed.

Indication and limits

The method comes into its own for patients with sufficient natural tissue and a subtle, harmonious wish for augmentation. For very large augmentations, more complex anatomical starting situations or patients with hardly any natural tissue, the classic procedure remains the medically superior choice. Which technique makes sense is decided by the findings – and the conversation.

What is still open

Reliable long-term data are still pending. Establishment Labs currently relies on data from a comparable technique in its own portfolio – the Mia method – in which clinical studies documented neither capsular contracture, rupture nor notable inflammatory reactions. A dedicated three-year study on Preservé is under way. As with any breast surgery, risks remain, and in rare cases it may be necessary to switch to a conventional technique during the procedure. This is part of the informed consent discussion.

Assessment

Preservé is a carefully developed technique that meets the needs of many patients better today than classic methods. Whether it holds what it promises in the long term will be shown by ongoing research. Until then, the following applies: the indication decides – and honest advice is the best starting point.
