Who is gynaecomastia surgery suitable for?
The surgery is particularly useful for men who see no improvement in their chest shape despite exercise and a healthy diet. After the procedure the chest looks firmer and more masculine, which improves overall body image.
What is gynaecomastia?
Gynaecomastia is a condition in which male breast tissue is enlarged. This can give the chest a feminine appearance, also known as “male breasts”. The change occurs on one or both sides and, unlike “false” gynaecomastia (pseudogynaecomastia/lipomastia) caused by excess fat tissue, has to be treated differently.

Consultation & preparation
Before treatment, our specialist plastic surgeons advise on all methods and their risks. Preparation includes, among other things, a medical history, blood tests and, where necessary, an endocrinological assessment.
Surgery day
On the day of treatment, depending on the findings and method, the procedure is performed under either local anaesthetic or general anaesthetic.
Aftercare & healing
After the surgery, swelling, bruising and a feeling of pressure may occur; these subside or ease over the course of weeks. Patients can usually return to everyday life after about a week, although strenuous physical activity should be avoided for 4 to 6 weeks.

Gynaecomastia is usually treated surgically, particularly when the condition is more severe or no medical cause is found that can be treated by other means.
Surgical treatment is usually carried out under general anaesthetic and consists of two main methods:
- Liposuction for pseudogynaecomastia
- Tissue removal for gynaecomastia
Depending on the findings, a combination is also possible. In more pronounced cases, a breast lift may also be indicated.
Although true gynaecomastia can often be felt by touch, an ultrasound examination is usually carried out as well. This tells us reliably whether enlarged glandular tissue or excess fat tissue (false gynaecomastia) is involved.
We also give recommendations and guidance on preparing for the gynaecomastia procedure.
In some cases a mammogram (X-ray examination) is also recommended to rule out the possibility of breast cancer. Breast cancer is, however, very rare in men.
The surgery usually takes one to two hours, so you can usually leave the clinic the same day. For more extensive procedures we advise an overnight stay at the clinic.
A compression garment should also be worn during healing. It supports the skin and stabilises the shape.
Final scar formation can also sometimes take several months. It is important to look after the scars properly to support healing:
Will scars remain visible after gynaecomastia surgery?
As the procedure involves only small incisions, the scars are often barely visible once healed. The surgeon also places the incisions strategically in inconspicuous places, such as around the nipple or in the armpit, to keep the visibility of the scars to a minimum. It also helps to protect the scars from sun exposure for 6 months.
In cases of true gynaecomastia, where the enlarged breast tissue is due to increased glandular development, tissue removal, often combined with liposuction, is necessary.
This removes excess breast gland tissue, which is responsible for the volume of the breast. It is done through an incision at the edge of the nipple or in the armpit, to leave scars that are as inconspicuous as possible. In some cases the surgeon may also adjust the areola to achieve as harmonious and natural a result as possible.
Depending on how much the skin of the chest has loosened, a breast lift may also be an option. This can be the case, for example, after significant weight loss, with pronounced gynaecomastia or, more generally, with weak and correspondingly sagging tissue.
Here, in addition to connective and/or fatty tissue, excess skin is also reduced under general anaesthetic. In cases of marked sagging, the nipple can also be moved back to a more natural position. For this the surgeon usually places the incisions around the nipple and vertically downwards or along the inframammary fold.

Gynaecomastia surgery makes sense above all when the enlarged breast has persisted over a longer period, a change of lifestyle and drug therapy bring little or no result, and wellbeing is affected.
Yes, the result of gynaecomastia surgery is generally permanent as long as body weight remains stable. However, renewed enlargement may occur through hormonal changes or weight gain. In addition to weight management, an endocrinological (hormonal) check and follow-up of the patient is therefore essential.
Suitable candidates for gynaecomastia surgery are men who suffer from a permanently enlarged breast, have a stable BMI and have no serious health problems. A medical examination also helps to rule out other causes such as hormonal disorders.
Light activities are possible again approx. 3–7 days after gynaecomastia surgery, while physically strenuous work should be paused for at least 2–4 weeks.
