A rounder face can look youthful – but not every fullness is wanted. If you want to permanently slim the midface and define the facial contour more clearly, you can have cheek fat removed. Targeted reduction of the deep-lying buccal fat pad – known as buccal fat removal – gives more structure to the lower cheek area and a more elegant, more expressive profile.
Buccal fat removal is a surgical procedure for the targeted removal of the deep-lying cheek fat – the so-called buccal fat pad. The aim is to slim the face visually and bring out the natural contour in the middle third of the face more strongly. The method is particularly suitable for people with genetically rounder or softer facial features, in whom the lower midface appears dominant and a slimmer, more defined look is wanted. As an alternative to surgical removal, a visual slimming of the lower half of the face can also be achieved through targeted midface slimming treatments – for example with anti-wrinkle injections to reduce the masseter muscle, or contouring with hyaluronic acid. These methods are non-invasive and particularly suit patients who do not want surgery.
Result and longevity
The result develops gradually over a few weeks, once the postoperative swelling has gone down. The final effect is permanent, as the removed fatty tissue does not grow back. Facial expression is fully preserved, and the result looks harmonious and individually tailored – not “hollow” or overcorrected.
Important aspects of aftercare
In the first days after the procedure there may be slight swelling, a feeling of pressure or temporarily restricted chewing. Intense physical exertion and hot food should initially be avoided. Wound healing usually proceeds quickly and without complications. As there is no external skin scar, no scar care is required – good oral hygiene, however, is essential to avoid infection.

Buccal fat removal in Berlin: what to expect
The treatment is carried out as an outpatient, under local anaesthetic or, on request, twilight sedation. Through a small incision of approx. 1–2 cm on the inside of the cheek – so without an external scar – the buccal fat pad is carefully exposed and removed in a precisely measured amount. The procedure takes about 30 to 45 minutes per side. As access is through the oral mucosa, the skin structure of the face is fully preserved. The volume in the lower cheek area is reduced, creating a clearer contour between the cheekbone, cheeks and jawline. The face looks slimmer, more elegant and more expressive – without appearing unnatural.
