Breast augmentation can be performed using either implants or the patient's own fat. Both procedures have their own advantages and disadvantages, which are taken into account depending on the individual needs and wishes of the patient.

Breast augmentation with silicone or saline implants

Breast augmentation with implants is the most common method and is preferred by many women worldwide. In this procedure, silicone or saline implants are inserted into the breast to increase volume and improve the shape of the breasts.

Depending on the patient's anatomy and the desired results, the surgeon decides whether to place the implant under the mammary gland or under the chest muscle.

The size and shape of the implant are determined based on
    • the desired breast size,
    • body shape and
    • chosen based on the patient's aesthetic goals.

silicone implants

These implants offer a particularly natural feel and realistic appearance. They are available in various shapes and sizes, allowing them to be tailored to the individual needs of the patient. Most modern silicone implants are filled with a special gel that provides a comfortable, soft feel and meets the requirements of natural breast tissue.

saline implants

These implants consist of a silicone-coated shell filled with sterile saline solution. Saline implants offer a safer option because, in the event of a leak, the body absorbs the fluid. However, these implants may feel slightly firmer and less natural than silicone implants.

The surgical procedure is usually performed under general anesthesia and takes between 1 and 2 hours.

Healing after breast augmentation can take around 2 to 4 weeks, during which time special compression garments must be worn to stabilize the implants and support healing.

Mild pain, swelling, and bruising may also occur in the first few days after the operation. However, these symptoms usually subside quickly. Strenuous physical activity and lifting heavy objects should be avoided for the first few weeks after the operation.

Risks

Like any surgery, breast augmentation carries risks that should be considered. A major concern with implants is often that they will rupture and "leak." Modern cohesive silicone gels ("Gummy Bear") do not literally leak, as the gel is more stable in shape, but even here, the gel can escape from the silicone shell. In the case of an intracapsular rupture, it remains in the surrounding scar capsule and does not spread freely throughout the body. However, if it migrates beyond this (extracapsular), it can spread to the surrounding tissue or lymph nodes. In both cases, surgery should be performed to remove it. Breast augmentation with autologous fat, also known as fat transfer, is a less invasive method of breast augmentation in which fat is removed from another area of the body using Vaser liposuction and injected into the breasts. This procedure is often preferred by women who do not want implants but would like to benefit from liposuction. Common fat removal sites are the abdomen, thighs, or hips.

The advantage of breast augmentation with autologous fat lies in the natural texture of the tissue, which results in a softer and more authentic appearance. In addition, no foreign materials such as implants are required, which eliminates the risk of rejection or capsule formation. However, it is important to note that not all injected fat is permanently retained - usually some of the fat is absorbed by the body. Therefore, a follow-up treatment may be necessary to stabilize the desired result.

The procedure is usually performed under general anesthesia or twilight sleep and takes about 2 to 3 hours, depending on the extent of liposuction and fat transfer or injection into the breast.

However, with this method, liposuction must be considered a separate process, which means additional recovery time for fat removal. It is important to take care of the treated areas during healing and to have regular follow-up appointments with the surgeon to monitor the healing process.

The process of breast augmentation with autologous fat in Berlin

The process comprises several steps:

1. Fat removal:
Excess fat is removed from the desired areas of the body using liposuction. This is usually done through small, inconspicuous incisions.

2. Fat processing:
The extracted fat is cleaned and processed before being injected into the breasts.

3. Injection of the fat:
The prepared fat is injected in small amounts into the breast tissue to shape the form and increase the volume.

Scarring and aftercare for breast augmentation with implants and autologous fat

Both procedures usually leave small scars, but these are placed in such a way that they are as inconspicuous as possible.

In breast augmentation with implants, incisions can be made either along the crease under the breast, around the nipple, or in the armpit. With the autologous fat method, however, the scars are minimal and limited to small extraction sites where the fat was suctioned out and small injection points.

The final result can therefore usually only be assessed after several months, once the swelling has completely subsided and the tissue has stabilized.
