Many people wish to emphasize their body contours, be it through more volume or a more harmonious shape. A BBL with autologous fat offers a natural solution, as the body's own fat is harvested and processed from other areas such as the abdomen, hips or thighs and then injected into the buttocks. This results in a natural, harmonious augmentation and shaping.
Treatment procedure
BBL with autologous fat is performed in several steps. First, excess fat is removed from areas such as the abdomen, hips, thighs or back using gentle liposuction. The extracted fat is then purified and specially prepared to maximize the survival rate of the fat cells. The fat is then precisely injected into the buttocks, where it is placed in different layers to achieve a harmonious, aesthetic result.
The entire procedure usually takes two to four hours and is performed under general anesthesia or twilight sleep. Immediately after the treatment, the buttocks are swollen and the final result is visible after around three to six months, when the transplanted fat has stabilized.
Result and durability
As some of the injected fat cells are broken down again by the body, the final result is only visible after a few months. In the long term, the surviving fat is permanently retained, provided that the body weight remains stable. As this is the body's own tissue, the buttocks feel completely natural after the healing phase.
Important aspects of aftercare
After the operation, you should refrain from sitting for around two weeks so as not to jeopardize the growth rate of the fat cells. A special compression garment helps to minimize swelling and provide optimal support for the newly formed tissue. Physical activity is possible again after four to six weeks.

Which body parts are suitable for fat harvesting for a BBL?
Typically, fat is removed from the abdomen, hips, thighs, back and/or flanks to create a slimmer silhouette at the same time.
How long does the result of a BBL with autologous fat last?
Some of the injected fat is broken down in the body, but the permanently healed fat cells remain. The final result is visible after around 3 to 6 months.
Is a BBL with autologous fat safe?
Yes, if it is performed by an experienced specialist. As the body's own fat is used, there is no risk of allergic reactions or rejection.
What alternatives are there to BBL with autologous fat?
Alternatives are buttock implants or hyaluronic acid fillers. Implants provide a firmer shape, while fillers are suitable for a subtle volume increase. At Innerform, we do not perform BBLs with implants as the procedure is associated with increased complications.
How painful is a BBL?
The pain is individual, comparable to severe muscle soreness. The first few days are sensitive, but painkillers help to alleviate the discomfort.
When will I be able to sit and sleep on my back again?
During the first 2 to 4 weeks, direct pressure on the buttocks should be avoided to ensure optimal healing of the fat cells.
How long does it take to heal after a BBL?
Most patients can resume light activities after 1 to 2 weeks. Complete healing takes about 3 months.
What follow-up care is important after a BBL?
Wearing compression pants for 6 to 8 weeks, lymphatic drainage to reduce swelling and avoiding direct pressure on the buttocks are crucial.
How much fat survives after BBL surgery?
Around 60-80% of the transplanted fat cells survive in the long term, the rest are broken down by the body in the first few weeks.
Can I do sport after the operation?
Light exercise is permitted after 2 weeks, intensive training should be avoided for at least 6 weeks.
Can I combine a BBL with other treatments?
Yes, it is often combined with VASER® liposuction, J-Plasma for skin tightening, cellulite treatments (e.g. Emtone) or body fillers for targeted contours.
