Breast Augmentation Without General Anesthesia: What the Preservé Method Has Changed—and What It Still Needs to Prove
For several years now, there has been growing interest in aesthetic surgery in procedures that are less invasive, involve less downtime, and still deliver clinically convincing results. Motiva’s Preservé method is one of the most sophisticated attempts to date to meet these requirements in breast augmentation.
The Technology
The basic principle differs fundamentally from traditional procedures. Instead of cutting through tissue with surgical instruments, an inflatable balloon is inserted through a narrow, blunt-tipped channel. This gently pushes the surrounding breast tissue aside—thus creating space for the implant without involving the muscle or disturbing the tissue. The implants themselves, the Motiva Ergonomix, are designed to be particularly soft and flexible, which is what makes placement through a small incision possible in the first place.
The procedure takes about an hour, is performed under local anesthesia, and postoperative pain is usually well managed with ibuprofen. Many patients are able to move around again by the evening; driving is possible after 48 hours, and light exercise after one week.
What this means clinically
The reduced tissue trauma has tangible benefits: shorter healing times, less pain, and a faster return to daily life. For women with demanding professional or family commitments, this fundamentally changes how they approach the decision to undergo surgery. One patient who has particularly stayed in our memory: She had wanted breast augmentation for years—but general anesthesia was a line she never wanted to cross. With Preservé, that barrier was removed.
Indications and Limitations
This method is most effective for patients who have sufficient natural tissue and are seeking a subtle, harmonious enhancement. For very large augmentations, more complex anatomical conditions, or patients with very little natural tissue, the traditional procedure remains the medically superior choice. The appropriate procedure is determined by the medical evaluation—and the consultation.
What remains to be done
Reliable long-term data are not yet available. Establishment Labs currently relies on data from a comparable procedure in its own portfolio—the Mia method—for which clinical studies have documented no cases of capsular fibrosis, rupture, or significant inflammatory reactions. A separate three-year study on Preservé is currently underway. As with any breast surgery, risks remain, and in rare cases, it may be necessary to switch to a conventional procedure during surgery. Classification
Preservé is a meticulously developed procedure that better meets the needs of many patients today than traditional methods.
